Who Is CFC For?

CFC is designed to help people who need assistance with daily living so they can continue living in their homes and communities. This program serves:

  • Children with Disabilities: Young individuals who need extra help with daily activities or specialized support.
  • Seniors (65+): Older adults who require assistance with tasks like bathing, mobility, or household chores.
  • Adults with Disabilities: Individuals who benefit from in-home care to maintain independence and safety.
  • Blind Individuals: Support for personal care, mobility, and essential household tasks.

By offering these services, CFC helps people avoid unnecessary institutional care and remain where they feel most comfortable.

How the Program Works

The process for receiving CFC services is simple:

  • Application: Eligible individuals can apply for CFC through Health First Colorado online or by calling their local Health First Colorado office.
  • Assessment: Once an application is submitted, a professional assessment is completed to identify needs and determine eligibility.
  • Authorization: Services are authorized based on the assessment to ensure the right care plan is created.
  • Care Services Begin: After approval, Voyager Home Health Care provides high-quality, compassionate care directly in the home.

Eligibility Requirements

To qualify for CFC services, individuals must:

  • Be eligible for Health First Colorado under the State Plan.
  • Meet institutional level of care requirements, meaning care is needed that would otherwise be provided in a nursing home or hospital.
  • Complete a state-prescribed eligibility assessment confirming the need for assistance.
  • Meet financial qualifications for full Medicaid benefits.

These requirements ensure that CFC reaches the people who need it most.

How Payments Work

At Voyager, we handle caregiver payments directly, so families never need to manage timesheets or payment systems. Caregivers are paid on a biweekly schedule, providing consistency and reliability. This approach allows families to focus on care and well-being instead of administrative details.

Implementation Timeline

Community First Choice will be fully available in Colorado beginning July 1, 2025. Current waiver members will transition during their Continued Stay Review from July 2025 to June 2026. For new members, services will be available starting July 2025. By fall of 2026, all eligible individuals will receive services through the CFC program.

Additional Services and Benefits

CFC includes a Wellness Education Benefit, which helps members improve health, manage chronic conditions, and maintain quality of life. Children may also receive services through the CwCHN Waiver, which offers respite care, case management, therapies, and in-home support services under CFC.
